This was not really a leak per se, but we were referencing the textures for
longer than intended. If textures were set via llvmpipe_set_sampler_views()
(for fs) and then picked up by lp_setup_set_fragment_sampler_views(), they
were referenced in the setup state. However, the only way to unreference them
was by replacing them with another texture, and not when the texture slot
was replaced with a NULL sampler view. (They were then further also referenced
by the scene too which might have additional minor side effects as we limit
the memory size which is allowed to be referenced by a scene in a rather crude
way.) Only setup destruction (at context destruction time) then finally would
get rid of the references.
Fix this by noting the number of textures the last time, and unreference
things if the new view is NULL (avoiding having to unreference things
always up to PIPE_MAX_SHADER_SAMPLER_VIEWS which would also have worked).
Found by code inspection, no test...

but I accidentally pushed an older version of the patch in the end...
Also prettify slightly. And make some notes about the confusing and useless

ARB_fragment_layer_viewport requires that if a fs reads layer or viewport
index but it wasn't output by gs (or vs with other extensions), then it reads
0. This never worked for llvmpipe, and is surprisingly non-trivial to fix.
The problem is the mechanism to handle non-existing outputs in draw is rather
crude, it will simply redirect them to whatever is at output 0, thus later
stages will just get garbage. So, rather than trying to fix this up (which
looks non-trivial), fix this up in llvmpipe setup by detecting this case there
and output a fixed zero directly.
While here, also optimize the hw vertex layout a bit - previously if the gs
outputted layer (or vp) and the fs read those inputs, we'd add them twice
to the vertex layout, which is unnecessary.
And do some minor cleanup, slots don't require that many bits, there was some
bogus (but harmless) float/int mixup for psize slot too, make the slots all
unsigned (we always put pos at pos zero thus everything else has to be positive
if it exists), and make sure they are properly initialized (layer and vp index
slot were not which looked fishy as they might not have got set back to zero
when changing from a gs which outputs them to one which does not).
This fixes the failures in piglit's arb_fragment_layer_viewport group
(3 each for layer and vp).

Multiple scenes per context are meant to be used so a new scene can be built
while another one is processed in rasterization. However, quite surprisingly,
this does not actually work (and according to git log, possibly never did,
though maybe it did at some point further back (5 years+) but was buggy)
because we always wait immediately on the rasterizer to finish the scene when
contexts (and hence setup/scene) is flushed. This means when we try to get
an empty scene later, any old one is already empty again.
Thus using multiple scenes is just a waste of memory (not too bad, since the
additional scenes are guaranteed to be empty, which means their size ought to
be one data block (64kB) plus the size of some structs), without actually
really doing anything. (There is also quite some code for the whole concept of
multiple scenes which doesn't really do much in practice, but keep it hoping
the wait-on-scene-flush can be fixed some day.)

GL (3.0) allows you to clear individual color buffers in a fb. In fact
for fbs containing both int and float/normalized color buffers this is
required (because the clearing values are otherwise undefined if applied
to all buffers). The gallium interface was changed a while ago, but llvmpipe
ignored it (hence doing such individual clears always resulted in clearing
all buffers, plus some assorted asserts due to the mixed fbs).
So change the clear command to indicate the buffer to be cleared. Also, because
indicating the buffer to be cleared would have made lp_rast_arg_cmd larger
which is unacceptable (we're trying to shrink it some day) allocate the clear
value in the scene and just pass a pointer.
There's several advantages and disadvantages here:
+ clearing individual buffers works (we could also actually bin such clears now
if they'd come through clear_render_target() if the surface is in the current
fb, though we didn't do this before for the single rb case and still don't try).
+ since there's one clear per rb, we do the format conversion in setup rather
than per bin. Aside from the (drop in the ocean...) performance advantage this
means that clearing to very small values (that is, denormal when converted to
the format) should work for small float (fp16 etc.) formats, as the util code
couldn't handle it correctly before (because cpu denorms are disabled when
executing the bin commands, screwing up the magic conversion and flushing
the values to 0, though this was not verified).
- there's some overhead for traditional old-style clear-all MRT cases, since
there's one rast clear command per rb instead of one for all rbs.
This fixes https://bugs.freedesktop.org/show_bug.cgi?id=76976.
v2: get rid of the ugly manual memcpy stuff and just use union util_color.
This is 32 bytes instead of 16 but as the allocation is per scene we can live
with those additional 16 bytes (and the additional 128 bytes in the setup
context), which makes the code much more obvious. Suggested by Brian.

With this patch, generate_fs_loop will clamp any fragment shader depth writes
to the viewport's min and max depth values. Viewport selection is determined
by the geometry shader output for the viewport array index. If no index is
specified, then the default viewport index is zero. Semantics for this path
can be found in draw_clamp_viewport_idx and lp_clamp_viewport_idx.
lp_jit_viewport was created to store viewport information visible to JIT code,
and is validated when the LP_NEW_VIEWPORT dirty flag is set.
lp_rast_shader_inputs is responsible for passing the viewport_index through
the rasterizer stage to fragment stage (via lp_jit_thread_data).

Nowadays -1 for slots means that the semantic is not present, so
we need to store it in a signed variables, otherwise <0 comparisons
are pointless. Fixes

Draw module can decompose primitives into wireframe models, which
is a fancy word for 'lines', unfortunately that decomposition means
that we weren't able to preserve the original front-face info which
could be derived from the original primitives (lines don't have a
'face'). To fix it allow draw module to inject a fake face semantic
into outputs from which the backends can figure out the original
frontfacing info of the primitives.

OpenGL doesn't support this but d3d10 does.
It is a bit of a pain as it is necessary to keep track of queries
still active at the end of a scene, which is also why I cheat a bit
and limit the amount of simultaneously active queries to (arbitrary)
16 (simplifies things because don't have to deal with a real list
that way). I can't think of a reason why you'd really want large
numbers of overlapping/nested queries so it is hopefully fine.
(This only affects queries which need to be binned.)
v2: don't copy remainder of array when deleting an entry simply replace
the deleted entry with the last one (order doesn't matter).

Mostly just make sure the layer parameter gets passed through to the right
places (and get clamped, can do this at setup time), fix up clears to
clear all layers and disable opaque optimization. Luckily don't need to
touch the jitted code.
(Clears invoked via pipe's clear_render_target method will not work however
since the pipe_util_clear function used for it doesn't handle clearing
multiple layers yet.)

add assertion for impossible layer specification for surface.

If we're drawing to a surface that's 2048 x 2048 pixels or larger there's
danger of fixed-point overflow in the triangle rasterization code. That
leads to various rendering glitches.
Rather than implement some intricate changes to the rasterization code,
simply subdivide triangles into smaller subtriangles to avoid the issue.
Only do this when the drawing surface is larger than 2048 by 2048.

Split the sampler interface to use separate sampler and texture (sampler_view)
state. This is needed to support dx10-style sampling instructions.
This is not quite complete since both draw/llvmpipe don't really track
textures/samplers independently yet, as well as the gallivm code not quite
using the right sampler or texture index respectively (but it should work
for the sampling codes used by opengl).
We are however losing some optimizations in the process, apply_max_lod will
no longer work, and we potentially could end up with more (unnecessary)
recompiles (if switching textures with/without mipmaps only so it shouldn't
be too bad).
v2: don't use different callback structs for sampler/sampler view functions
(which just complicates things), fix up sampling code to actually use the
right texture or sampler index, and similar for llvmpipe/draw actually
distinguish between samplers and sampler views.
v3: fix more of PIPE_MAX_SAMPLER / PIPE_MAX_SHADER_SAMPLER_VIEWS mismatches
(both in draw and llvmpipe), based on feedback from José get rid of unneeded
static sampler derived state.(which also fixes the only 2 piglit regressions
due to a forgotten assignment), fix comments based on Brian's feedback.

Before this we only supported user-based constant buffers.
First, we basically plumb pipe_constant_buffer objects through llvmpipe
rather than pipe_resource objects.
Second, update llvmpipe_set_constant_buffer() and try_update_scene_state()
so they understand both resource- and user-based constant buffers.
The problem with user constant buffers is the potential for use-after-free,
as seen in some WebGL tests. The next patch will flip the switch for
resource-based const buffers.

This required an update for the query storage in llvmpipe, there
can now be an active query per query type, so an occlusion query
can run at the same time as a time elapsed query.
Based on PIPE_QUERY_TIME_ELAPSED patch from Dave Airlie.

Now dead code.
Also had to remove the show_tiles/show_subtiles because now the color
buffers are always stored in their native format, so there is no longer
an easy way to paint the tile sizes.
Depth-stencil buffers are still swizzled.

This branch implemented dual representations of texture/drawing surfaces:
one in the conventional linear layout and the other the tiled layout which
is used by the fragment shader pipe. Per-tile flags indicate the layout
of each image tile. In many situations this lets us avoid converting
image data between the two layouts.

llvmpipe: checkpoint WIP: overhaul texture/surface mapping
Conversion between tiled and linear surfaces is working everywhere now.
The LP_TEXTURE_READ/READ_WRITE/WRITE_ALL flags let us avoid unnecessary
image layout conversions.

nvfx: fix for gallium-resources
It seems to work with basic applications but almost surely needs more work.
In particular, it probably shouldn't use PIPE_BUFFER_USAGE_* flags
and should use PIPE_TRANSFER_* in several places.
Also, we probably don't want the vtable indirect calls and that ought
to be replaced with something better instead.

nouveau: convert nvfx and nv50 to pipe_resources
Compile tested only.
This was a deeper change than I was hoping for, due to the
layering of the pipe_texture implementation in each driver on
top of a shared pipe_buffer implementation in the shared code.
Have modified the shared code to act as a set of convenience
routines operating on nouveau_bo objects.
Each driver now uses the u_resource_vtbl technique to split the
implementation of pipe_resources between the existing miptree code
for textures and a new, minimal buffer implementation in each
driver.
Eventually these should be combined, not least because APIs are now
allowing things like binding buffer resources as textures and render
targets.

r300: convert to pipe_resources
Do a very shallow conversion - basically keeping the existing
buffer and texture code intact and using a vtbl struct
inside our resource struct to select between the two implementations.
The buffer and texture treatments could be further merged without
much effort, but try to keep the existing code working at this point.

We weren't saving the per-scene texture references at the right point.
Fixes piglit cubemap segfault. The segfault resulted from referencing
texture memory which was prematurely freed because of a missed reference
count.
Fixes fd.o bug 27276.
