The array format is what the GL API wants (fixing texbos on
big-endian), and matches directly to gallium's corresponding array
format. The only driver exposing A8L8 was radeon/r200 in big-endian,
where the HW's underlying format was trying to read as array and we
needed to flip things around to make our packed format come out right
(note that while the radeon format tables had both AL and LA,
ChooseTextureFormat would only pick one of them based on endianness).
v2: Don't make r200/radeon use endian swaps.
v3: Rebase on dropping the r200 _be/_le format table removal patch

debugoptimized builds don't define NDEBUG, but they also don't define
DEBUG. We want to enable cheap debug code for these builds.

A number of drivers report useful debug/perf information accessible
through GL_ARB_debug_output and with debug contexts (i.e. setting the
GLX_CONTEXT_DEBUG_BIT_ARB flag). But few applications actually use
the GL_ARB_debug_output extension.
This change lets one set the MESA_DEBUG env var to "context" to force-set
a debug context and report debug/perf messages to stderr (or whatever
file MESA_LOG_FILE is set to). This is a useful debugging tool.
The small change in st_api_create_context() is needed so that
st_update_debug_callback() gets called to hook up the driver debug
callbacks when ST_CONTEXT_FLAG_DEBUG was not set, but MESA_DEBUG=context.

The new driver hook has x/y/zoffset and width/height/depth parameters
for the new glGetTextureSubImage() function.
The meta code and gallium state tracker are updated to handle the
new parameters.
Callers to Driver.GetTexSubImage() pass in offsets=0 and sizes equal
to the whole texture size.

If a user set MESA_INFO and the OpenGL application uses a
3.0 or later context then the MESA_INFO debug output will have
an error when it queries for extensions using the deprecated
enum GL_EXTENSIONS. Passing context argument allows code
to return extension list directly regardless of profile.

This is an OpenGL ES specific extension. External textures are textures that
may be sampled from, but not be updated (no glTexSubImage* and etc.). The
image data are taken from an EGLImage.

Nothing in Mesa supports color-index textures, and most of the other
infrastructure that could allow such support has already been removed.
This puts the final nail in the coffin.
Also clean out some GL_COLOR_INDEX comments in formats.c.

Remove the inclusion of fpu_control.h from compiler.h. Since Bionic lacks
fpu_control.h, this fixes the Android build.
Also remove the sole use of the fpu_control bits, which was in debug.c.
Those were brianp's debug bits, and he approved of their removal.
