This `gen_scrn_dispatch.pl` has never existed, in the sense that NVIDIA
never published it. There have been a number (6) of commits to fix
various things in there over the years, and never anything from NVIDIA.
For all intents and purposes this file is hand-written and
hand-maintained, and we're on our own.
Let's make this clear by removing this misleading comment.

glXGetDriverConfig parameters do not provide a context to dynamically
check for the presence of the function, so the dispatcher directly calls
glXGetDriverConfig, but in non-dri builds dri_glx.c didn't provide
glXGetDriverConfig.
This change make it just return NULL in that case.

Together with some fixes to xdriinfo this fixes xdriinfo not working
with glvnd.
Since apps (xdriinfo) expect GetDriverConfig to work without going to
need through the dance to setup a glxcontext (which is a reasonable
expectation IMHO), the dispatch for this ends up significantly different
then any other dispatch function.
This patch gets the job done, but I'm not really happy with how this
patch turned out, suggestions for a better fix are welcome.

would only sometimes work.

Rather than checking if the function name maps to a valid entry in the
respective table, just create a dummy entry at the end of each table.
This allows us to remove some unnessesary "index >= 0" checks, which get
executed quite often.

With reference to the libglvnd branch:

This is a squashed commit containing all of Kyle's commits, all but two
of Emil's commits (to follow), and a small fixup from myself to mark the
rest of the glX* functions as _GLX_PUBLIC so they are not exported when
building for libglvnd. I (ajax) squashed them together both for ease of
review, and because most of the changes are un-useful intermediate
states representing the evolution of glvnd's internal API.
