This bumps the limit to the number of sets to 32, now that
we have proper support for it. It also uses 1u in a few places
to make things a bit safer.

Needed if we want to allow them taking more than 64 KiB. The calculations
of these already used 32 bits.

This squashes all the radv development up until now into
one for merging.

This requires llvm 3.9 and is in no way considered
a conformant vulkan implementation. It can run a number
of vulkan applications, and supports all GPUs using
the amdgpu kernel driver.
