Changed FetchTexel() function pointer arguments.
Implemented glGetTexImage(format=GL_COLOR_INDEX).
Changed _mesa_unpack_depth_span() args.
Minor changes/clean-ups in mtypes.h.
Histogram counter component sizes were wrong.
