blorp/clear: Add a binding-table-based CCS resolve function
v2:
- Do layered resolves.
(Jason Ekstrand):
- Replace "bt" suffix with "attachment".
- Rename helper function to prepare_ccs_resolve.
- Move blorp_params_init() into helper function.
