Many timest the titles are not really duplicate. The detection logic only
checked that they used the same sectors of the disc, but there are many other
things that can make the titles different.

the theora encoder requires it's "frame_width" and "frame_height" to
be aligned to 16pix.
This also updates to using the new API introduced in theora 1.0.
This is an interim fix. Later we will adjust our video buffers to actually
have 16pix aligned data in them.

combinations of options possible, as well as a debug mode that overlays the combing mask on the output. A full explanation of the new modes can be found at the top of libhb/decomb.c. The new default parameter string is: 7:2:6:9:80:16:16:10:20:20:4:2:50:24:1
- Cleans up some compiler warnings for decomb.
- Partially fixes mcdeint in decomb. I would not suggest using it yet, but it's getting piped the right data, it no longer outstrides its memory, and lavc finally fixed their Snow bug that broke iterative ME). Seems to work well on fully interlaced material, with the spatial metric (2nd paramter) set to -1. Output with hybrid material and when actually decombing is...poor, but these are, hopefully, surmountable problems.
- Lays some groundwork for bobbing.

Read and log the region mask of the DVD drive.
We get the occasional linux user that has an unset region. Logging
the region will help isolate the problem more quickly.

- there are now two buttons to add subtitles
one button adds normal subtitles from the source.
the second adds SRT subtitles.
- when an SRT subtitle is selected in the list, the available controls change
SRT controls are: Language, Character Codeset, File, and Time Offset (ms)
- A combo entry box is used for character code. A subset of the most common
character codes is available in the popup. Other codes can be manually
entered in the entry box.
- The last used SRT file directory is remembered.
- Remove the debug printf eddyg left in decsrtsub.c

Attempting to seek one sector forward doesn't work. dvdnav rounds the seek
request *down* to the nearest vobu. So the seek actually goes backwards.

- fixed ports.c to use a more portable method of getting integral pthread_t representation; resolves mingw crash.
- added GCC attribute to generate compiler warnings for invalid usage of hb_log, hb_deep_log and hb_errror;
see new macro HB_WPRINTF(s,v) in common.h.
- fixed various invalid usage of above functions on osx i386/x86_64, and mingw.

- subtitles were not being added to list of subs to scan because the wrong
config was being checked for the forced flag. needed to use
job->select_subtitle_config instead of subtitle->config
- scr code in reader was preventing the majority of subtitle packets from
reaching decvobsub. made decvobsub more robust by adding ability to
re-sync in the event of lost packets. made scr recovery initialize
itself even in the event that it sees audio or subtitles before seeing
video.

render video output to the free list causing hangs and/or garbage output.
- since frame rate conversion moved from sync to render, sync has to use input (title) frame rate, not output (job) frame rate.
- get rid of 'too many frames' check in sync. it can't be done here because it causes reader to deadlock and it belongs in the dvd reader anyway (only dvd input can loop).

use hb_subtitle_add() to add output tracks.

the entire subtitle stream from the same source track on a second
output track
- LinGui: make subtitle tab resemble functionality of the audio tab
any track can be added multiple times with different settings.

- when doing an indepth scan, do not scan CC tracks
- separate subtitle configureation attributes into separate
hb_subtitle_config_t. Add an instance of this to hb_job_t for
setting the attributes of the subtitle found through an indepth scan
- Add a default_track flag to hb_subtitle_config_t that tells the muxer
that the track should be flaged as the default. muxmkv uses this.
- When an indepth scan is complete, check to see if the autoselected
subtitle matchces (by id) one of the manually selected subtitles.
If a match is found, the autoselected subtitle with all the attributes
the user assigned to it replaces the manually selected subtitle.
- LinGui: Add "Default" column to subtitle tab. This is a radio that lets
the user choose which subtitle should be displayed by default.
