The default chapter start or end was used if one but not the other of
start-at/stop-at were used.
Also don't overwrite current pass and pass_count status in hb_state_t
while searching for the position to start encoding.

Theora can create 0 length output. These 0 length frames indicate
duplicate frames. So we can't use 0 length buffers to indicate the end
of the stream.

This global was shared between the CLI and libhb and used as a back door to
force scan and encode passes to use the same ffmpeg context for hardware
decoding. Aside from the fact that this context sharing should not be necessary
and needs fixing, this information belongs in the hb_handle_t that is shared
between the scan and the encode. So put it there and make sure the hb_handle_t
get propagated to where the flag is needed.

Simplifies the WinGui.
This also changes how jobs are processed. Creating the sub-jobs for
multiple passes is delayed until after scanning and immediately before
running the job.
Working status has also changed. Sub-job passes are identified in status
with an ID that allows the frontend to definitively identify what pass
is in progress.

Dolby Digital Plus (E-AC-3), Dolby TrueHD and FLAC can now be passed through without re-encoding. They aren't covered by Auto Passthru yet, however.
In addition, encoding to E-AC-3 is now possible.

There are several changes to job and title structs that break
current windows interop code. The interop code should be changed
such that it only uses json APIs. So if there is any missing
features (or bugs) in these APIs, please let me know.

If an SSA starts before and ends after the subsequent SSA, the subtitles
were improperly merged resulting in timestamp issues.

-1 is not a good value as a flag for invalid timestamps.
There are cases where small negative timestamps are useful.
So this eliminates a potential ambiguity.

FDK is non-experimental, so if it's compiled in, it will be selected over the libavcodec encoder, which is what we want to use for consistency (quality hardly matters when encoding silence).

stop time properly.
I also did not handle the last subitle properly. It would have been lost.

mp4 timed text does not support subtitles that overlap (in time).
So this patch detects these overlaps and merges subtitles when
appropriate.

if an error happens during this period, the hang would happen.
check w->done in these loops to break out in case of error.

When we insert silence to maintain sync, we typically need to insert about
70ms. But the default flac frame duration is very large (>=96ms). This is
causing us to overshoot the target and we immediately drop a frame. Dropping
a frame undershoots and we insert a frame... ad infinitum.
This patch changes the behavior of silence insertion for non-passthru audio.
Instead of inserting full frames of silence, the last "frame" inserted may be
a partial. This lets us insert a finer grained silence duration and eliminates
the problem with large flac frames.

Cleans up several several unavoidable memory leaks caused by old api.
Clearly separates titles from jobs. Titles are set during scan and
never modified now.
Since titles are immutable, this lead to API some changes. For example,
We were setting chapter names in the title from the front ends. Now
these get set in the job.
These new APIs allow us to start moving away from our use of title->job.
Eventually, I would like to eliminate title->job completely, but the
mac ui is too tightly tied to using this field to allow removing it
at this time. So there is temporarily a convenience function used
only by the mac ui that allows it to continue using title->job and also
use the new APIs.
New APIs:

Initializes a new job with default settings from the title.

Same as hb_job_init(). For use by win Interop lib.
void hb_job_reset( hb_job_t * job );
Convenience function for the MacUi.
Clears audio, subtitle, and filter lists. The macui still uses
title->job because it is so intricately tied to it. So I created
this convenience function that it can call after adding a job.

Releases the job an all resources it contains.
void hb_job_set_advanced_opts( hb_job_t *job, const char *advanced_opts );
Makes a copy of "advanced_opts" and stores in job.
Freed by hb_job_close().

Makes a copy of "file" and stores in job.

sync needs the precise duration of a frame of audio and has
no way to compute it for passthru audio. So add a duration
member to hb_buffer_t and set it in the audio decoders.
This should eventually supersede hb_buffer_t member "stop"
since it provides redundant information.
